It’s programmed to work like this: you drag and drop the master anywhere on your screen. When you click on the comment text (that can be edited as you wish), the form pops up. You can connect your form to your datamasters’ fields with simple drag and drop, as also can add or delete any field you need to.

How to install: download the file, go to Justinmind Prototyper and click on the downward directed arrow in your Masters panel.
